MyMdBand is an internet/intranet based service, which allows You to electronically store, edit, upload and retrieve critical emergency medical that may influence the treatment provided by an emergency medical care administrator. Your data is accessed via the www.411.md website. The www.411.md website can be accessed via a standard browser or by scanning an identification device, such as a bracelet or pendant, with a QR code provided by MyMDband. MyMdBand also allows You to purchase online the unique MyMdBand bracelet, accessories and other products and services at https://shop.mymdband.com (referred to collectively as the "Services").
In order to provide You the Services, we collect Personal Information when You register with us, make purchases, activate Your account with us, and when You store Your health information and records on our Website. We will not collect any Personal Information about You unless You provide it to us voluntarily.
When You purchase a MyMdBand subscription, You will be asked to register with our Website. If You register with our Website, we ask for information such as Your full name, date of birth, email address, and ask You to create a password.
Payments made to purchase a MyMdBand identification device and subscription are done by using a credit card or through PayPal. Credit card transactions are handled by third party financial institutions and their vendors and contractors, who receive credit card numbers and other Personal Information from us to verify the credit card numbers and process transactions. We may also, provide non personally identifiable information to others based upon the medical information stored in our database such as how many people in the database suffer from high cholesterol.
Other than as stated above, we do not disclose Personal Information, or any other information, to any outside party without specific authorization from You.
